In addition ot being a cost-effective option for receiving health care services, home health promotes quicker recover and improves the quality of life for both patient and their family.
1. Beneficiary's medical condition restricts the ability to leave home without the assistance of another individual or without the assistance of a supportive device.
2. The beneficiary's medical condition should restrict the ability to leave home.
If, however, the the beneficiary chooses to leave home frequently, even though leaving home requires a considerable and taxing effort, the beneficiary would not be considered homebound.
1. Beneficiary leaves home only to receive medical treatment that generally cannot be provided in the home.
2. Leaves the home infrequently; for short periods for non-medical purposes or to attend a religious service or unique event such as a funeral or graduation, and these absences do not indicate the beneficiary has the ability to obtain health care provided outside the home, then the beneficiary may still be considered homebound.
To determine whether the beneficiary has the general ability to leave the home, look at the beneficiary's condition over a period of a time rather than short periods within the home health stay.
Advanced HealthCare Services' primary focus is to provide individuals and their families with safe and effective quality home health care services. Care will begin within 48 hours after the request is received. We can also arrange for rental and/or or purchases of wheelchairs, walkers, special beds, or any assistive devices which may be needed. Also upon request we can coordinate other services such as finding homemakers, companions or sitters.
Advanced Healthcare Services, LLC nurses oversee patient progress, and assist the patients and caregivers to understand:
Advanced Healthcare Services, LLC evaluates and develops a plan of treatment and establishes home programs to improve mobility, transfer skills, ambulation, gait, balance and coordination.
Advanced Healthcare Services, LLC employs state certified nurse assistants who are trained to assist patients with their personal care, hygiene, grooming, and activities of daily living.
Advanced Healthcare Services, LLC will evaluate and provide treatment for patients to improve their communication skills. Speech Therapists will also evaluate the patient's ability to swallow safe.
Advanced Healthcare Services, LLC provides social workers that can provide knowledge and information about community resources. As well as social and emotional support for patients and caregivers.
Advanced Healthcare Services, LLC evaluates and provide a plan of care in the use of therapeutic , creative self-care activities of daily living and adaption to their living environment.
Advanced Healthcare Services, LLC upon your request, can coordinate other services such as finding homemakers, companions or sitters.
